MOST GP APPOINTMENTS, REGARDLESS OF BOOKING METHOD WILL BE A TELEPHONE CONSULTATION IN THE FIRST INSTANCE, YOU WILL BE INFORMED SPECIFICALLY IF IT IS A FACE TO FACE APPOINTMENT. 

IF YOU ATTEND THE PRACTICE, IT IS STILL REQUIRED FOR YOU TO WEAR A FACE MASK UNLESS YOU ARE EXEMPT, AND TO ATTEND ALONE UNLESS YOU NEED ASSISTANCE.

NEW PATIENT REGISTRATION / INFORMATION

If you would like to register with us, please visit our website and follow instructions to register online or speak to a member of reception staff.  Our catchment area is predominantly the E6 postcode.

For all patients, once registered, your named accountable GP is Dr MSJ Khan; however, you can see any of our GPs.

You will also be given ONLINE ACCESS to book appointments and order repeat medications – please speak to reception regarding further access to your medical records.

We contact patients through telephone calls, text messages, letters, emails – please let reception know if you have any objections with this.

 

DISABLED PATIENTS

Our premises accommodate disabled persons.

 

OPENING HOURS

Monday to Friday: 8am-6.30pm

 

APPOINTMENT TIMES / BOOKING SYSTEM

Appointments vary on a daily basis; they are available between 8.30am – 6.30pm.

Appointments with a Practice Nurse can be booked at any time during opening hours.

Appointments with a GP can be booked as per below; we will try to offer you an appointment with your preferred doctor but it will depend on their availability:

Appointments for the next working day can be booked online or by calling the surgery from 10am until fully booked.

Advanced appointments are available 2 – 30 days in advance and can be booked online or by calling the surgery at any time until fully booked.

Online Consult is available for patients to use at any time, for non-urgent medical concerns.  You can complete the form online by visiting our website and you will be contacted within 48 working hours.

 

EVENING & WEEKEND APPOINTMENTS

Appointments can be booked at local surgeries 7 days a week 8am-8pm.  We can book these for you or you can call 020 7511 2075.  Appointments are available with GPs, Physician Associates, Nurse Practitioners, Practice Nurses and HCAs.

 

HOME VISITS

Please call the surgery as early as possible.  Your call will be logged by the Receptionist and the Doctor will phone you back as soon as they can, if a home visit is not required you may be advised to attend the surgery.  Remember that most problems are best dealt with at the surgery.

 

WHEN THE SURGERY IS CLOSED

If you have a medical concern that cannot wait for the next working day, please telephone 111.  Your call will be screened for the most appropriate care (please note calls are monitored for inappropriate use).  You may also visit your local pharmacy for advice or treatment under the:

 

SELF HELP

If you have minor health conditions such as those listed below, you can go to your local pharmacy or visit www.patient.co.uk where you can get detailed health advice as well as information on treating symptoms at home or taking over the counter medications, Sore Throat, Colds, High Temperature, Hay Fever, Headaches, Indigestion, Athlete’s Foot, Head lice, Teething, Insect Bites and Stings, Mouth Ulcers, Back Pain and Soft Tissue Injuries.

 

ACCIDENT & EMERGENCY

Please do not attend the Accident and Emergency services at the hospital unless it is absolutely necessary.  Please contact the surgery.  Don’t just go to A&E you could get quicker treatment closer to home’.

 

PRACTICE NURSES (Jessica, Dorothy, Grace, Jennifer)

You may make an appointment with the nurses who will offer you expert advice and care for the following:   Blood Test results, Dressings, Vaccinations, Cervical Smears, Contraception, Lifestyle Advice, Immunisations, Ear Syringing, Asthma and Diabetes etc.

 

HEALTHCARE ASSISTANTS (Beena – Level 3 with additional training, Lauren – Level 3, Maggie – Level 4)

Blood Tests, Flu, Shingles, Pneumonia, B12 Injections, New Patient Health Checks, Cholesterol, BP Checks, NHS Health Checks, ECGs, Elderly and Carer Health Checks.

 

ADDITIONAL CLINICAL ROLES

Clinical Pharmacists can review medication and carryout basic health reviews.

Physician Associate can assess medical health conditions and liaise with a GP in regards to issuing medications.

 

PRACTICE MANAGEMENT

Management ensures the smooth running of the practice. If you have any concerns or complaints about the practice, please ask to speak to the reception supervisor (June) in the first instance who will try and resolve your concerns, if she is unable to help, she will speak to the manager.   Further information on the complaints procedure is available on line or at reception.

 

RECEPTIONISTS                                                                   

We have friendly and helpful receptionists who will try and help you with any queries that you may have.  Please be patient with them.

 

REPEAT PRESCRIPTIONS

These will only be issued to patients where it is clinically indicated and authorised by the Doctor.  Please nominate your chosen pharmacy so that prescriptions can be sent electronically.  This enables easy access.  Please allow 48 hours.  You may be asked to see the Doctor before a repeat prescription is issued. Prescriptions can also be ordered online.

 

TEST RESULTS

We will contact patients if a follow up appointment is required once the results have been received. If you wish to discuss your results with a clinician, then please book a routine appointment.  It is vital that you give updated telephone numbers and change of addresses as quickly as possible.

 

TERMINATION OF PREGNANCY

Termination enquiries are redirected to the British Pregnancy Advisory Service on 03457 30 40 30 or www.bpas.org

 

MATERNITY SERVICES

We provide joint services with the hospital and we have a midwife who attends the surgery twice a week. Once your pregnancy is confirmed, you will need to attend the surgery to collect an antenatal self-referral form; this then needs to be taken to the hospital, along with photo ID to book your appointments with the antenatal team.

 

FAMILY PLANNING

Family planning service is provided.  These include pre-pregnancy advice, pill prescribing, emergency methods and natural family planning.

 

HEALTH VISITOR

The health visitor supports families with children under the age of 5.  They have walk-in clinics at a number of local surgeries.  Please speak to reception for more information or call 020 8218 7420.

 

POST NATAL & BABY CHECKS

Once your baby is born, mums and babies need to be checked by the GP from 6 weeks. Please call to book an appointment.

 

CHILDHOOD IMMUNISATIONS

For advice on immunisations, please speak to the Health Visitor or Practice Nurse.  Parents/guardians should accompany all children for their immunisations; otherwise they may not be seen if they attend with another relative.

 

NEWHAM COMMUNITY DRUGS TEAM

If you are concerned about yourself or a member of your family you can call the drugs team on 020 8536 2121 for help and information.

 

TRAVEL VACCINES

Travel vaccines are provided by the Practice Nurses.  Please make an appointment 6 weeks before you plan to travel.  There is a fee for vaccination certificates and certain travel vaccines if required, but we do not offer ACWY for travel purposes, please enquire at your local travel clinic.

 

 FLU INJECTIONS

These are provided between September to March for patients who meet the NHS criteria i.e. 65 years old and over, those with chronic diseases, carers, pregnant women etc..  Please ask at reception for more information.

 

CONFIDENTIALITY / INFORMATION SHARING

We maintain strict confidentiality and only disclose information with your consent or if we are required to do so by law.

We comply with the Data Protection and Access to Medical Records legislation.  Identifiable information about you may be shared with others in the following circumstances; to provide further medical treatment for you, for example, District Nurses, Health Visitors, Hospitals etc. and Child Protection cases, when we have a duty of care to others.  Anonymised patient data may be given to local and national levels to be used for e.g. study, plan services etc. Our Privacy Notice is viewable on our website or ask at reception.  If you do not wish for your data to be used in this way, then please let us know.

 

ZERO TOLERANCE

Both staff and patients are treated with mutual respect. We abide by the NHS guidelines on Zero Tolerance; aggression/violence/threats towards any members of this surgery will not be tolerated.

 

PATIENT INFORMATION RIGHTS & RESPONSIBILITIES

Patients are expected to attend for their appointments on time.

If you are unable to keep your appointments, please call the surgery as soon as possible (preferably 2hrs prior) to cancel your appointment.

Patients have the right to expect doctors and nurses to provide them with a satisfactory standard of care.

You have the right to access the services we provide according to your clinical need.

 

NON-NHS SERVICE

Some services are not provided by the NHS; therefore a small charge will be levied. These include Medical Examinations, Insurance Forms, Private Certificates and Letters etc. Please ask at reception.

 

 HOSPITAL SERVICE FOR OVERSEAS VISITORS

Where a GP refers overseas visitors to the hospital, the hospital may charge them for treatment, particularly if they have not been a resident in the UK for the previous 12 months, even if the GP has treated them on the NHS.

 

MEDICAL CERTIFICATES

Doctors are not required to issue statements for absence from school or for periods of incapacity for work lasting up to 7 days.  You may obtain a self-certificate from the DSS or your employer.  If you require a private certificate, this will be given at the doctor’s discretion and there will be a charge.
